Carers

My name is Paula and I’m the Carer Champion for the practice. My role is to support patients who provide care for a family member, friend or neighbour and to ensure you’re aware of the help available.

If you are a carer, you can contact us by phone, letter, or email to let us know who you care for or simply to tell us that you’re a carer, we do have a form available to compete for ease. Once this is recorded, we can offer additional support such as flu vaccinations for carers and added flexibility with appointments.

If you are a carer for one of our patients but registered with a different GP practice, please also inform your own GP so they can record your caring role and offer any support you may be entitled to.

You can self-refer for support at any time using the following services:

• Lancashire County Council Adult Carer Support
• Young Carers Support

Each of these services provides guidance, practical help, assessments, and further links to local organisations.
